Pressure-controlled intermittent coronary sinus occlusion impulse system

The Pressure-controlled Intermittent Coronary Sinus Occlusion Impulse System (also known as PISCO Impulse System which is a registered trademark), is a procedure utilized to enhance restoration of blood flow to the microcirculation in the heart tissue following a myocardial infarction.〔(【引用サイトリンク】url=http://www.vasculardiseasemanagement.com/news/miracor-successfully-completes-first-german-patient-using-picso-system-treat-severe-heart-attac )
==History==
A myocardial infarction, or heart attack, is typically due to the build-up of plaque along the coronary arteries of the heart, causing them to narrow and prevent proper blood supply.〔Van de Werf, F, Bax, J, Betriu, A, Blomstrom-Lundqvist, C, Crea, F, Falk, V, Filippatos, G, Fox, K, Huber, K, Kastrati, A, Rosengren, A, Steg, G, Tubaro, M, Verheugt, Weidinger, F, Weis, M. (2008). “Management of acute myocardial infarction in patients presenting with persistent ST-segment elevation”. Eur Heart J 29 (23): 2909-2945.〕 A plaque may break off and form blood clots that disrupt blood flow and inhibit oxygen delivery to the heart leading to a MI.〔 A lack of oxygen and nutrient supply along with a reduction in waste removal in tissue damage that may lead to cell death and myocardial dysfunction.
Coronary angioplasty has been used as a way to restore blood flow following an MI to the heart in order to prevent excessive damage to the tissue.〔 Despite the restoration of blood flow that occurs with this procedure, approximately 30-40% of patients still have poor health outcomes.〔van de Hoef, TP, Mohl, W, Khattab, AA, Wykrzykowska, JJ, Piek, JJ. (2011). “Cardioprotected Percutaneous Coronary Intervention- A Case Study in a Stable Angina Patient”. Interv Cardiol 6 (2): 186-189.〕 This is most likely the result of inadequate blood flow to the smaller arteries and veins of the heart which play a vital role in the delivery of blood and nutrients to the heart muscle.〔van de Hoef, TP, Nolte, F, Delewi, R, Henriques, JPS, Spanna, JAE, Tijssen JGP, Siebes, M, Wykrzykowska, JJ, Stone, GW, Piek, JJ. (2012) “ Intracoronary Hemodynamic Effects of Pressure-controlled intermittent Coronary Sinus Occlusion (PICSO): Results from the First-In-Man Prepare PICSO Study”. J Interv Cardiol 25 (6): 549-556.〕 Reduced blood flow causes scar tissue to develop within regions of the heart leading to reduced heart function and the potential for heart failure to develop.,.〔〔
